WELCOME TO THE OMSD NEW STUDENT ENROLLMENT APPLICATION
This website is the first step in enrolling a new student in school. We will collect important information about the student, including emergency contacts, medical details, and language preferences. We kindly request that when completing the online enrollment form, you refrain from using all capital letters and instead use standard capitalization. Once completed, the student's information will be electronically submitted to the school.

Required Documentation for Student Enrollment - Please read entire page and scroll down to the bottom for more instructions.
Address Verification (submit document)
Parent/Guardian must provide one of the following documents to verify residency:
* A current utility bill (e.g., gas or electric bill)
* A mortgage statement
* A rental aggreement
Birth Documentation (submit document)
Parent/Guardian must provide one of the following to verify the student's birthdate:
* A birth certificate
* A baptismal certificate
* A passport
Immunization Records (submit document)
Parent/Guardian must provide up-to-date immunization records, including the following required immunizations:

Entering 7th Grade:
For students entering 7th grade, the parent/guardian must provide immunization records showing all immunizations required for Transitional Kinder and Kinder requirements, plus:

Health Inventory Form (fill in template & submit document)
The Parent/Guardian must complete a Health Inventory Form for each student. Once completed, please follow these steps:
1. Click here to download the form
2. Fill out the form with the required information.
3. After completing the form, save a copy to your computer or device.
4. Once saved, attach the completed Health Inventory Form to the New Student Enrollment application online.
Additional Documentation (if applicable)
Physical Exam (print copy and submit completed form)
While not required, it is recommended that children entering Transitional Kinder, Kindergarten, First Grade, (who have not previously attended school), or those transferring from out of the state or out of the country, submit a completed physical exam form. This form should be signed and dated by a physician on a CHDP (California Health and Disability Prevention) form and stamped with the clinic's seal.
Please note that a physical exam is required for children entering Preschool.

Oral Health Assessment (print copy and submit completed form)
Children entering Transitional Kinder or Kindergarten must have a dental exam before enrolling. The oral health assessment form must be signed and dated by a dental professional and stamped with the clinic's seal.

Transitional Kinder/Kindergarten Questionnaire (fill in template & submit document) - must be completed for all students enrolling in TK or Kindergarten.

Enrollment is not complete until all required documents are reviewed and approved by the school office staff.
